This Veterinary Agreement is between [Operator Business Name] ("the Operator") and [Vet Practice Name] ("the Practice").
Scope of services
The Practice agrees to provide veterinary services to animals in the care of the Operator, including:
— Emergency treatment out of hours
— Routine health checks and vaccinations
— Telephone advice during normal hours
— Annual review of preventative healthcare plans
Operator obligations
The Operator agrees to:
— Maintain accurate records of each animal's medical history
— Isolate any animal showing signs of infectious disease before contacting the Practice
— Pay invoices within 30 days of receipt
Fees
Fees are charged at the Practice's standard rates, reviewed annually.
Termination
Either party may terminate this agreement with 30 days' written notice.
